The following supporting documentation must accompany the completed account Application Form:

• Power of Attorney, if applicable
• Proof of address (Passport, Driving License, Utility Services Bills e.g. Gas / Electricity / Water, not less than 3 months old
• Proof of identity (Passport only)
• Bank statement, not less than 3 months old

For applications from outside the UK, please supply notarized/certified copies of these documents.
